Leadership Pharmacy, a new-practitioner leadership conference, will hold its 20th annual conference in 2008 at the Eagle Ridge Inn and Resort near Galena, Illinois. The Conference will provide an unequalled opportunity for pharmacists in their first ten years of practice (who have graduated before 1998) to develop organizational
skills which will enable both personal and professional growth. In recognition of the importance of this Conference to the development of future leadership for professional organizations, the Iowa Pharmacy Association and the Pharmacy Society of Wisconsin have agreed to help underwrite all lodging, meals and program costs for
each participant, making it an expense-free weekend of professional development.

Leadership Pharmacy combines professional and social functions over three days, allowing participants to network
with pharmacists from neighboring states during leadership programming, recreational activities, and issues discussions. Ten pharmacists will be selected from Wisconsin and Iowa (20 total) to attend the Conference.

Please consider recommending a pharmacist within his/her first ten years of practice to participate in Leadership
Pharmacy. It promises to be an exciting and fulfilling weekend! An application package including a detailed schedule

This Nomination Form is for those pharmacists who do not qualify for participation in Leadership Pharmacy, but wish to submit the name of a colleague who does qualify.
