- Excellent source of information and expert insight into optimal use of
antithrombotic and anticoagulant therapy.
Institute
for Healthcare Improvement (IHI) - Includes a variety of tools and tips
for standardizing anticoagulation management.

of Washington Medical Center Anticoagulation Services
- Popular site that includes a number of inpatient and outpatient protocols,
guidelines, patient information, and links to vitamin K content lists.
UW
Health Anticoagulation Services Website - University of Wisconsin site
has PDFs of algorithms for diagnosis and treatment of venous thromboembolism
(DVT and PE). Therapeutic protocols include heparin infusion, HIT, vitamin
K, factor 7 and ambulatory warfarin management. Patient education materials
are also included. This site will eventually be expanded to include other
anticoagulation protocols. Other UW protocols available by e-mail (see other
