Medication Reconciliation Project
This year the Pharmacy Society of Wisconsin (PSW) and the Wisconsin Hospital
Association (WHA) formed a project team to analyze the process of medication
reconciliation. One of the goals of the project team has been to develop and
disseminate medication reconciliation information and tools to our members
so that they can successfully perform this process.
The project team recognizes the diversity and individuality of each
institution. In their decision to assist members with medication
reconciliation, the team decided to recommend a list of key components that
they believe should be incorporated into any medication reconciliation tool
that is being used by an institution. A file listing the key components can
be viewed here. The project team
has also identified key
process components that you may wish to consider as you develop your
procedure for medication reconciliation at your institution.
In addition, we are happy to provide you with
examples of various medication
reconciliation tools that are currently being used around the State of
Wisconsin. The institutions listed on this Web site have provided these
tools specifically for your reference and review. Each institution and its
corresponding tool are listed along with a resource individual. If you have
any questions about a respective tool, please contact the resource
