Nomination for Cardinal Health Generation RX Champions Award

The Cardinal Health Generation Rx Champions Award was established in 2011 to recognize a pharmacist that has demonstrated a commitment to the mission of substance abuse education. The nominee must be a PSW pharmacist member.


  1. Community service
  2. Impact on the profession through commitment to prescription drug abuse and misuse education that is targeted to youth, their parents and the elderly
  3. Implemented prescription drug abuse and misuse education programs in their practice site and/or community that are informed by evidence-based research
  4. Participation in other professional activities


If you would like to nominate yourself or a colleague, please provide a nomination letter describing the most important achievements of the nominee and explaining why the nominee meets the criteria for the award. Additional letters are welcomed. Nomination materials must be submitted to sarahs@pswi.org no later than February 1. The 2015 award recipient will be recognized at the PSW Educational Conference on April 10.
