Ambulatory Care Pharmacy

Ambulatory Care Pharmacy is the provision of integrated, accessible health care services by pharmacists who are accountable for addressing medication needs, developing sustained partnerships with patients, and practicing in the context of family and community.

Best Practices for Ambulatory Care Practitioners

The following represents best practices for ambulatory care practitioners.Not every practitioner or practice setting will allow for incorporation of every one of these practices; however, they represent a goal for us all to strive towards.

  • Complete an accredited residency training program or equivalent post-licensure experience prior to entry into direct patient care practice
  • Obtain board certification once the clinical pharmacist meets the eligibility criteria specified by the Board of Pharmacy Specialties
  • Follow standard processes of care:
    • Assessment of the patient utilizing the electronic medical record
    • Evaluation of medication therapy
    • Development and implementation of a plan of care (either collaboratively or independently)
    • Follow-up evaluation and monitoring
  • Document care in the shared electronic medical record
  • Obtain organizational or state-based credentials and privileges
  • Practice under scopes of practice and/or collaborative practice agreements
  • Follow standard codes of professionalism and ethics
  • Participate in research, scholarship and/or education/mentorship
